Delete your myFHC account

For the myFHC app by Florida Healthcare Collective LLC, on Google Play and the App Store. You can delete your account yourself in the app, or ask us to do it.

Delete it in the app

  1. 1Open myFHC and sign in.
  2. 2Go to You → Security → Delete account.
  3. 3Confirm twice. Your account closes right away.

If you no longer have the app or can’t sign in, email our Privacy Officer at admin@flhealthcarecollective.com from the address on your account, with the subject “Delete my myFHC account”. We may ask you to confirm it’s you before we act, and we’ll reply when it’s done.

What happens when you delete

  • Your login is closed for good and every phone or computer signed in to it is signed out.
  • We stop sending you emails and text messages from myFHC.
  • If you signed up for FHC news, you are removed from that list.
  • If you also want the rest of your personal information removed (your profile, photo and what you added in the app), write to us as above. We’ll erase or anonymize it, except what the law requires us to keep.

What we keep, and for how long

Florida Healthcare Collective is a HIPAA-covered entity. Some records can’t be deleted on request: records of care you gave or received, credentialing records (licenses, certifications and the documents you submitted), and the audit trail of who accessed health information. We keep them for the period Florida and federal law require, then dispose of them securely. Our Privacy Policy and Notice of Privacy Practices explain this in full.
